Peter Attia trained as a surgeon and left the OR for a longer problem: how people actually die. Heart disease, cancer, neurodegeneration, metabolic collapse. He calls the preventive version Medicine 3.0. The book is Outlive. The podcast is The Drive. If you only remember one picture from him, make it the centenarian decathlon. Pick the physical jobs you still want at 80 (stand up off the floor, carry a suitcase, hike, pick up a child) and train those qualities now, while you still can add them.

Exercise sits at the top of his pile, and it is not close. VO2 max and muscle mass keep showing up in mortality data. In practice that looks like a lot of easy aerobic work (Zone 2, you can talk), a little truly hard work, heavy strength, and stability so the strength is usable outside a gym. Rucking, deadlifts, and a hill you do not enjoy. This is why he belongs with muscular fitness and longevity, not with a bottle of NAD.

Metabolic health is the floor under that. Glucose handling, visceral fat, ApoB (not the vanity LDL headline). Food, for him, is there to feed training and keep you from being metabolically broken. He has gone through public ketogenic chapters. The through-line is protein, muscle, and not pretending diet is a religion.

Sleep and emotional health got a whole section in Outlive because they wreck the training if you ignore them. Less gadget-friendly. More true.

His clinic is expensive and test-heavy. Most of the signal he cites is still boring: fitness, muscle, blood pressure, lipids, cigarettes, sleep. Extra labs help when they change a decision. They are a hobby when they do not. He is also more comfortable with drugs (statins, GLP-1s, aggressive lipid lowering) than a typical biohacking forum. That is a physician’s risk math, not a coupon for self-prescribing from episode 214.
